Two years after Luke Abrahams, 20, died on a hospital operating table, the coroner said new information had come to light in a report by the ambulance service.

A critical incident is the highest level of alert and means that additional steps are taken to ensure patient safety.

Pictures show the bus trapped underneath the bridge, with the entire roof ripped off. The incident happened on Sunday evening, involving a Stagecoach bus.
