Loose Women and Mind Lighten The Load Hero Award Nomination 2019 Terms and Conditions

Nominations open for the Lighten TheLoad Hero Award as promoted by Loose Women (the “Programme”) on Wednesday 25th September 2019.
Closing date and time:
Nominations for the Lighten The Load Hero Award close on Friday 11th October 2019 at 10:30am.
Eligibility:
Entrants (“You” / the “Nominator”) and the person nominated by You (the “Nominee”) must meet the following eligibility criteria:
a) Nominators and Nominees must be UK, Channel Islands or Isle of Man residents;
b) Nominators and Nominees must both be aged eighteen (18) years old or over as of 11th October 2019;
c) It will be your (the Nominator’s) responsibility to obtain permission from the Nominee to nominate them before submitting their nomination and to make them aware that their nomination is subject to these terms and conditions and the Promoter’s Privacy Policy and to further notify them that part of the prize is attending the Mind Media Awards to collect their awards and that they may be required to appear on the Programme or the Mind Media Awards livestream and/or other communications supporting the event;
d) Nominators and Nominees must not be employed or engaged by ITV (or any ITV group company)or Mind;
e) Nominees must agree to take part in any filming, broadcast and publicity related to the Mind Media Awards;
f) SuccessfulNominees must be available to travel to London to attend the Mind Media Awards ceremony to be held on Wednesday 13th November 2019 and attend theProgramme on Thursday 14th November 2019;
g) Nominators and Nominees must let the Promoter know if they have ever been convicted of a crime, if they are the subject of ongoing criminal proceedings or investigations, or if they have any County Court Judgments against them. Nominators and Nominees agree that Promoter has the right to do, or to engage third party organisations to do, background checks on them to verify the information that they provide in this regard (including but not limited to Disclosure & Barring Service checks or Disclosure Scotland checks carried out by third party organisations);
h) Disclosure of criminal convictions, current criminal proceedings or investigations and other matters including those that may cause potential disrepute, may not automatically exclude nominees from the selection process but the Promoter, theITV/Mind team and/or any judging panel shall be entitled to exercise their sole discretion in this regard and may invalidate a Nominee’s application or otherwise result in the termination of the Nominee’s participation in the Programmes or the Mind Media Awards. In all cases the decision of the Promoter, the ITV/Mind team and/or the judging panel(s) shall be final. If we collect information about criminal convictions, we will treat this information in accordance with the following Privacy Notice:https://itvstudios.com/privacy-notice/talent-contributors-audience;
i) If the Nominee is unable to participate in the process for reasons beyond their control, another person on their behalf may represent them in the process, at the Promoter’s discretion;and
j) The Promoter reserves the right to check that all eligibility conditions have been met at any time throughout the process.
What we are looking for:
The nomination is to put a spotlight on the people who are putting the principles of the Lighten the Load campaign into action every day, which include someone who went above and beyond with the little things in life that made things easier for You and continued to support your recovery from your mental health issues.
How to nominate:
To make a nomination, send an e-mail to Loose.Women@itv.com with“Mind Lighten The Load Hero” in the subject line of the email and the following information in the body of the email:
Your name and the name of your Nominee; Your telephone number; A headshot photo of the Nominee in colour and in JPEG format up to 10MB in size; and You must detail in up to 300 words who You are nominating, how the Nominee who went above and beyond with the little things in life that made things easier for You and continued to support your recovery from your mental health issues, who you think deserves to be recognised as the Lighten the Load Hero; (the “Selection Criteria”).
Any entry which is more than 300 words may not be considered or, at the producers’ discretion, only the first 300words of the entry may be considered.

Only one (1) nomination may be submitted per Nominator.
By submitting a nomination to loose.women@itv.com, you agree the following:
o you acknowledge that both You and the Nominee are aged 18 or over as of 11th October 2019;
o ifYou (the Nominator) are under 18 years old, you must have your parent or legal guardian’s permission to nominate and you must provide the name and daytime telephone number of your parent or legal guardian on request;
o you have read and accept the Terms and Conditions (https://www.itv.com/terms/participation-charity/itv-general-participation-terms) and Privacy Notice (https://www.itv.com/privacy);
o you have read and accept these Mind Lighten The Load Hero terms and conditions;
o that the Promoter and/or the Mind team may contact you and/or yourNominee in relation to such nomination;
o you have the explicit consent of the Nominee and any other person mentioned in your nomination to pass their details to us or, where the Nominee is under the age of 18 at the time of submission of the nomination, that you are the parent or guardian of that Nominee;
o with regards to the headshot photo of the Nominee:
§ you are the person who took the photo you are entering, or you have the permission of the person who took the photo to submit the photo as part of the nomination; and
§ if the Nominee in the photo is under the age of 18at the time of your submission of the nomination, you confirm that you are the parent or guardian of that Nominee; and
§ you have the consent of the Nominee to use the photo to enter the competition and to be used on Loose Women and/or feature at the Mind Media Awards show. If you do not have the appropriate consents, ITV reserves the right to disqualify you from the competition;
o you do not submit any material which in any way infringes the copyright or other rights of a third party, or which infringes any applicable laws. You hereby waive all moral and similar rights in your entry to the fullest extent possible and acknowledge that you will not receive any fee or royalty payment from the Promoter, producers or any third party for the consents and grants given by you in relation to your entry; and
o your entry does not contain any material which is in any way defamatory, obscene,unlawful, harmful, offensive or inappropriate or may have the effect of being harassing, threatening, abusive or hateful or that otherwise degrades or intimidates an individual or group of individuals on the basis of religion,gender, sexual orientation, race, ethnicity, age or disability or which refers directly or indirectly to alcohol or intoxication and must not infringe third party rights (for example, by including third party branding).
ThePromoter does not represent, undertake or warrant that it or the Mind team will contact all Nominators or Nominees. For the avoidance of doubt, if we suspect that You and/or your Nominee are not eligible to participate in this nomination process, we reserve the right to terminate or suspend their further participation. The Nominee must comply with any instructions and/or directions as given to them by the Promoter, their subcontractors and agents including without limitation in conformity with the Ofcom Broadcasting Code and all applicable rules, laws, regulations and guidelines.
Cost of nomination:
There is no charge for nominations submitted via email with the exception of your internet service provider's fees, where applicable.
Nomination selection process:
Nominations received by the closing date and time will be considered for the Lighten theLoad Hero Award. All entries will be checked by Loose Women’s production team to ensure that they meet all the eligibility criteria. All those which meet the stated eligibility criteria shall be read with the Loose Women production team choosing the shortlist of five nominees they consider best meet the SelectionCriteria. The judging panel (consisting of representatives from Loose Women,Mind and one independent judge) shall pick the winner from the shortlist.
One (1) judge will be nominated to be the head judge who will determine the outcome in the event of a tie or in the event of a technical failure or other circumstance beyond the reasonable control of the Promoter, which affects the outcome.
Filming:
Nominees must agree to make themselves available as and when requested by the Promoter for filming and/or other contact for the Mind Media Awards on Wednesday 13th November 2019 and for Loose Women on Thursday 14th November 2019 and/or such other dates as may be determined by the Promoter in their sole discretion.
Shortlisted Nominees must agree to be filmed and to provide their signature as proof of waiver and/or consent. Failure to provide such proof of waiver and/or consent from the Nominee may result in that Nominee not being able to participate further in the nomination selection process.
By participating in any filming or live performances on the Programme, the entrant and the Nominee agree that such footage may be published or broadcasted in any media in whole or in part by thePromoter including without limitation the Programme, without payment to the Nominee.
Winner’s Package
a) MindMedia Awards
The winner and the Nominator together with one (1) companion each (the “Winning Group”) will be invited to attend the Mind Media Awards ceremony inLondon on Wednesday 13th November 2019.
This part of the prize includes:
· Return standard class travel from the winner’s UK base to the London hotel booked by thePromoter for the Winning Group;
· 4tickets for the Winning Group to attend the Mind Media Awards;
· One(1) night’s accommodation for the Winning Group at a London hotel (with a minimum rating of three stars) on the night of the Mind Media Awards ceremony on a bed and breakfast basis based on two people sharing a double or twin room(subject to availability); and
· Return ground transfers from theLondon hotel to the Mind Media Awards ceremony and from the London hotel to ITVStudios for the Programme.
The winner will be contacted by the Promoter or the Mind team in advance of the event to arrange delivery of the Winner’s Package. The tickets to the Mind Media Awards will be available on the door.
It is the responsibility of the winner or the Nominator (as applicable) to inform thePrize Provider of any dietary requirements prior to attending the ceremony. It is the responsibility of the winner or the Nominator (as applicable) to pay for meals and drinks (other than stated above), any additional hotel charges, room service, mini-bar goods, souvenirs, telephone calls, excursions, treatments,additional travel and accommodation, taxes, insurance and any other expenses which may be incurred when attending the Mind Media Awards ceremony and thePromoter is not be responsible or liable to refund any such expenses.
It is the responsibility of the winner or the Nominator (as applicable) to inform thePrize Provider of any wheelchair or similar access requirements they may have and to adhere to any applicable health and safety guidelines.
Please note that the winner and/or Nominator will need a current and valid credit/debit card with sufficient available funds in order to check in at the hotel for security deposit purposes.
Companions of the winner and their Nominator must be aged eighteen (18) years old or over.
Please note this Winner’s Package is date specific and the winner, theirNominator and their companions must arrive in London on Wednesday13th November 2019.
The winner,their Nominator and their companions acknowledge that in the event that the MindMedia Awards ceremony does not take place as specified for any reason, thePromoter does not accept responsibility in these circumstances and no alternative Winner’s Package will be provided.
b) Cash
The winner will also receive a cheque for £500 (five hundred pounds) made payable in the name of the winner only. A signature may be required on delivery.
