Letby is serving 15 whole-life orders after she was convicted at Manchester Crown Court of murdering seven infants and attempting to murder seven others.

Dr Stephen Brearey said he "tried his best", but admitted at times it was not enough when raising concerns about Letby's behaviour to the hospital's management.

The Care Quality Commission were not told of concerns about unexplained deaths at the Countess of Chester Hospital’s neonatal unit where Letby worked.
