The Ministry of Defence has described the deaths of three SAS reservists - including one from Wiltshire - as "unacceptable". It follows an inquest which found "serious mistakes" over a training exercise, in which the men died of heat exhaustion.

The MoD has today published its response to comments by the coroner at the inquest into the death of Corporal James Dunsby, from Trowbridge, after the exercise on the Brecon Beacons in July 2013. Defence minister Penny Mordaunt says two new inquiries will look at safety lessons - and how bereaved families are supported.

She also admits that a new tracker system has not been working properly, is being upgraded, and that a new medical plan for training in hot weather will be reviewed daily.
