Speedboat's 'kill cord' was not worn
A report into a speedboat accident off Padstow has revealed that a television executive and his daughter died after he tried to make a fast turn.
All six members of the Milligan family were thrown out of the boat last May.
The report by accident investigators says the emergency "kill cord" to cut the engine had not been worn. The Chief Inspector of the Marine Accidents Investigation Branch says the cord was an "essential item of safety equipment".
