A grieving mother has described a report into her daughter's death as 'a disgrace'. 14-year-old Seren Bernard was in the care of Pembrokeshire social services when she was found dead in April 2012.
A report by the Local Safeguarding Children Board concluded that it may not have been possible to prevent her death.
But Seren's mother Sarah Pollock has dismissed the report and says she wants an independent review of the case. Kevin Ashford reports.
