A hearing has heard how a midwife failed to respond to a mother's repeated requests for an epidural.

Colleen Tyler's son Noah suffered brain damage during birth and died 10 months later.

Midwife Julie Louise Richards admitted allegations including failing to review Colleen's high blood pressure, but had denied a claim of ineffective communication.

Today the panel decided that she did not communicate effectively with Colleen as she was in the birthing pool.

The hearing as to whether Ms Richards is fit to practice continues.
