ITV Wales can reveal that warnings a maternity unit was on the brink of collapse were sent to the chief executive of Cwm Taf health board, two years before the current investigations into the deaths and still births of babies.

Allison Williams was sent emails - understood to detail staff concerns - back in 2016, but it was only near the end of 2018 that a review of maternity services was launched.

ITV Wales Health reporter, James Crichton-Smith, interviewed the Chief Executive. He asked her what she did, when she first received those warnings.
