Report finds 'serious and comprehensive breaches'

Here are five key steps the health board needs to take in light of the report into care on Tawel Fan ward of the Ablett Unit at Glan Clwyd Hospital.

It found 'serious and comprehensive breaches' in the level of care being given to some patients.

  • Tawel Fan ward should not be considered for re-opening until the final report from this investigation is received alongside a detailed review and understanding of any lessons to be learnt from the currently ongoing Tawel Fan mortality review.

  • Any new ward that is opened should be given a new name.

  • There should be a clear set of measurable standards set for the care of frail, older people with dementia across BCUHB.

  • A full public apology.

  • There should be a complete overhaul of the concerns, complaints and serious incident reporting structure.
