For months a man named Musa was sleeping rough in a bus shelter in Stoke Newington, east London.
Yesterday he died to the shock and sadness of those who passed him every day.
His death has sparked questions over whether he could have been given more help in his final weeks - and how, in a city like London, around 20 people a year still die on the streets of the capital.
Musa was well known in the area and locals had seen his refuse offers of help. But many felt somebody should have stepped in.
Hackney Council has launched an internal review of what happened but says Musa was repeatedly offered help with housing and his health by a range of organisations.
