Senior members of staff at the healthcare provider have said for the first time in 10 years that the death of Hayley's Brunt son could have been prevented.

Donna Ockenden says she's hearing new reports of mothers being shouted at by staff and some are being subjected to racism.

Senior midwife Donna Ockenden, who is leading the review, says 70 more families are due to be contacted in the coming weeks.
