Daughter left caring for siblings after mother's death

Holly Kitt's mother died following complications relating to pregnancy at Hull and East Yorkshire Women and Children's Hospital.

A letter has been sent to all health workers at the hospital admitting that the deaths of two mothers may have been avoided and telling to staff to ask themselves if they are in the right job.
