'I survived the Paddington rail crash'

On October 5 1999, a packed express train nearing Paddington collided head-on with another service that had just left the west London station, killing 31 people - including both drivers - and injuring a further 500 passengers.

Despite suffering horrific burns to her head and hands during the accident, commuter Pam Warren went on to be a leading light in a campaign to improve rail safety. However, in private, her life was falling apart.
