Seren Bernard's mother brands report 'a disgrace'

A mother has described a council report into her daughter's death as "a disgrace". 14-year-old Seren Bernard was in the care of Pembrokeshire when she was found dead.

I believe it was a failing to recognise her problems and put preventative measures in place that we ended up with this tragedy.

I believe that my daughter's death can only be a lesson. Please make it a lesson.

– Sarah Pollock, mother of Seren Bernard

A report by the local Safeguarding Children Board concluded that Seren's death may not have been prevented. Her mother Sarah Pollock has dismissed the report and says she wants an independent review of the case.
