Death of Pembrokeshire teenager Seren Bernard 'may not have been prevented'

The death of a 14-year-old girl from Pembrokeshire who was being looked after by social services may not have been prevented even if further steps had been taken to help her, a report has concluded.

Seren Bernard was found hanged in the Milford Haven area in April 2012. At the time of her death, she was living with foster carers and receiving support from social and mental health workers.

Her mother said Seren "was drawn to the wrong risks" later in her life.

The review made several recommendations "with the benefit of hindsight", including better coordination between the agencies looking after Seren.
